and will be billed at the usual and customary fees.

Early "Self Pay" Cash Plans
We offer special cash rates to clients who plan to pay the
whole fee before their babies are born.

  What is Included in Your Care
 
  All Prenatal Care – We see most women once a month for 28 weeks, every two
weeks until 36 weeks, then every week until the baby is born. If additional visits are
necessary, these are also included.

Complete Care During Labor & Delivery – We will stay with you as needed
throughout your active labor. Even in the event of a transport to the hospital, we
make every effort to be with you until your baby is born.

Postpartum Care Through 6 Weeks for Both Mother and Baby - This includes
home visits as needed in the first week and two office visits through 6 weeks.

Book - Except for women who start care late in pregnancy, every client will receive:
--The Natural Pregnancy Book: Herbs, Nutrition, and Other Holistic Choices

Binder - At your first visit, you will receive a binder filled with information concerning
all aspects of pregnancy,  birth and the postpartum period.

Complimentary Photo Session - Clients will receive a newborn (baby only)
session, along with two 8x10 portraits (two poses). Baby photography sessions are
held at my home about four times per year. Does not apply for Medicaid, Late Care
or Special Circumstance Discounts.

Vaginal Birth after Cesarean (VBAC) Client Care – In addition to the office and
home visits listed above, all VBAC clients will receive counseling to cover any
specific needs and preparation needed for a successful VBAC birth.

Newborn Screening – The test is done between 3 and 7 days.
 
     
  What is Not Included in Your Care
 
  Birth Kit – Order forms for your birth kit are included in your client notebook. You
need to order your kit early enough to have it available for your home visit at 36
weeks. This birth kit contains the consumable items we use for the birth (sterile
gloves, bed pads, etc). The price for the basis kit is currently around $75.

Additional Birth Supplies – In addition to the birth kit, you will need a few other
items for the birth (such as juices, towels, etc). You probably already have most of
these things around your home. A list is provided in your notebook.

Childbirth Education Classes - Parents are encouraged to take the Hypnobabies
childbirth preparation classes, taught by Carolyn at the Birth Wellness Center
(s
tarting this summer, 2008). Bradley childbirth preparation is another option for
parents, taught by area instructors.

Ultrasounds – We do not routinely order ultrasounds. Therefore when an
ultrasound is ordered, the cost is extra. Fees usually range from $150 - $225.
Some places accept insurance as well as Medicaid.

Lab Work – Labs are not included in the global fee, including the Standard lab
and any additional lab services that may be required.

Herbs, Vitamins and Medications – You will be asked to purchase and take some
good prenatal vitamins throughout your pregnancy. Sometimes we recommend
herbs for different situations. If you ever need any medications such
as antibiotics or Rhogam, (for mothers with RH- blood) this will be an additional
expense.

Physician Consult - Should you need a physician referral, you will need to pay for
his or her services separately.

Hotel Expenses For The Midwife, If More Than 50 Miles Away – If you live
more than 50 miles from Chattanooga and do not have an extra room with a bed and
an extra bathroom to accommodate us during our stay with you, then we ask that
you provide hotel accommodations. If the distance to your house hinders us from
taking a break at home during a long early labor (as we would if you lived in town), or
if we are too exhausted to safely travel on the road after a long birth, we will choose
to stay in the hotel. If it turns out that we do not need the hotel room, the money of
course is yours to keep. Please discuss specific arrangements with us prior to your
birth if you can not offer us an adequate room in your home and you live more than
50 miles away.

Mileage - There is $200.00 additional charge if you live more than 50 miles from my
office.
